OBTAINING AN INDIANA TEACHING LICENSE

(July 2009)

To apply for an Indiana Teaching License you must have:

  1. Met all requirements for an undergraduate degree and successfully completed a teacher education program
  2. Successfully completed Praxis I and Praxis II (subject area test)
  3. Successfully completed an Adult/Child CPR course and have valid certification through one of the approved providers listed at: http://www.doe.state.in.us/dps Course must include Heimlich maneuver.

Must have Visa or MasterCard

►►You still need to report your Praxis II scores and your CPR verification to IUB. When registering for Praxis II, list IUB as a recipient of your scores. Our recipient code is 1324.

►►A copy of CPR verification (front and back) MUST be sent to our office-- (NOT TO THE STATE)-- as a word attachment, OR through the mail, OR FAXED to:

We receive the application electronically, verify everything and electronically return it to the state. The state will print the license and send it through the mail to the address you listed on the application.

PAPER LICENSING APPLICATION:

  • Submit State Form 9331 - from Application Quick Links select "In-State Graduate, Original License"
  • Submit a valid CPR certification: All new applicants for an Indiana teaching license must have successfully completed an Adult/Child CPR course which includes the Heimlich maneuver. The most common way to obtain the proper CPR course is through a local Red Cross Chapter, through the American Heart Association or through any approved provider at: http://www.doe.in.gov/dps/cpr_certification.html
  • Submit a money order made payable to the State of Indiana for $35.00
